- MRCP uses heavily fluid-weighted MRI sequences that make bile itself glow bright white, so the ducts appear in detail without any dye injected into them.
- MRCP detects stones in the main bile duct with about 90–95% sensitivity, but stones smaller than roughly 5–6 millimeters are its best-documented blind spot.
- Most centers ask for a 4–6 hour fast so the gallbladder stays full of bile and stomach fluid doesn’t outshine the ducts on the images.
- The scan itself runs 20–45 minutes with several 15–20 second breath-holds, and many MRCP exams need no contrast injection at all.
- MRCP can show findings suspicious for bile duct, pancreatic, gallbladder, and ampullary cancers — including the ‘double duct sign’ — but confirming cancer always requires a tissue sample.
Advantage:
- An MRCP (magnetic resonance cholangiopancreatography) test is a non-invasive MRI scan that creates detailed pictures of the bile ducts, gallbladder, and pancreatic duct without a scope, incision, or radiation. The scan itself usually takes 20 to 45 minutes, most centers ask you to fast for about four to six hours beforehand, and it has largely replaced diagnostic ERCP because it carries far fewer risks.
